Many serious illnesses can develop quietly, producing only subtle symptoms before more obvious signs appear. Pancreatic cancer is one example because the pancreas sits deep behind the stomach, making early changes difficult to detect. As the disease progresses, however, it may trigger unexpected symptoms elsewhere in the body. Researchers have found that unexplained swelling in the legs, known as edema, can sometimes be an early warning sign, especially when it appears suddenly, affects one leg more than the other, or persists without a clear cause.

One reason for this link is the increased risk of blood clots. Pancreatic cancer is strongly associated with deep vein thrombosis (DVT), a condition in which a clot forms in a deep vein, usually in the leg. Symptoms may include swelling, pain, warmth, or skin discoloration. Doctors have long recognized that unexplained blood clots can occasionally signal an undiagnosed cancer, a phenomenon known as Trousseau syndrome, first described in the 19th century.

Scientists now understand that pancreatic tumors release substances that make the blood more likely to clot. Tumor cells produce large amounts of Tissue Factor, a protein that activates the body’s clotting process, while tiny particles carrying this protein circulate through the bloodstream. Many pancreatic tumors also release mucins, which interact with blood cells and platelets to encourage clot formation. Because blood naturally flows more slowly through the veins of the legs, these clots often develop there, causing swelling and inflammation.

Studies show that pancreatic cancer carries one of the highest risks of cancer-related blood clots, with an estimated 20% to 40% of people with advanced disease experiencing a clotting event. Doctors use tools such as the Khorana Risk Score to identify patients at greatest risk, and pancreatic cancer is automatically classified as high risk. Research also suggests that early clot formation may be linked to more aggressive forms of the disease, making unexplained leg swelling a symptom that should never be ignored and should be evaluated promptly by a healthcare professional.
